Has anyone tackled this yet? Which decoder, any surgery required?

That is not too hard, though it usually requires hard wiring. I have seen replacement board decoders, but they are almost as much trouble as hardwiring. In fact I just installed one a couple of weeks ago. Solder the red and black wires to the brass wires that go from end to end on the factory board, unclip the motor leads from those brass wires, solder the motor output wires to the motor leads, and run a piece of electrical tape or kaptan tape at theends of them so they don't touch track powered wires. The engine will now run on DCC. If you want the headlight to be controlled by the decoder, isolate the bulb from those brass wires and solder the bulb leads to the front headlight output wires on the decoder. A Digitrax DH123 will suffice for a decoder, though it is not the only brand or decoder that will work.

As long as the DH123 will fit easy enough, I do not want to do a total rebuild

The DH123 will fit easily enough, or you can use a Digitrax DN135D. The DN is about a third the size of the DH123, but does the same thing. MSRP is $19. Wiring colors are the same as the larger decoder, and it fits in a much smaller space. Here is a photo of the easiest way to hard wire either decoder. I have pulled back a little of the electrical tape and removed the decoder itself (I left the wiring) so you have less clutter in the photo:

Your better of with a light board replacement . Cut the light tubes as i beieve there is only one central bulb on existing and configure LEDs for both ends. Make numbers on computer for number board . Way easier removing all the existing board stuff. Pickups to both outer end tabs end of board, LEDs without resistors on each end inner tabs as most light board decoder replacements now have resistors on board. Motor wires to center edgeof board.
